The office is at 10209 N 35th Ave Ste 129B, Phoenix, AZ 85051, on 35th Avenue between Dunlap Avenue and Peoria Avenue, near Cortez Park. From Interstate 17 use the Dunlap exit (207) or the Peoria exit (208) and head west about a mile. Parking is on site.

What should I bring to a walk-in visit?
Bring a photo ID, your insurance card if you have one, and a list of any medicines you take. If a tooth was knocked out, bring the tooth in milk or in your own saliva, and keep it cool on a hot Phoenix day. If a crown fell off, bring the crown.
